大鼠坐骨神经缺损自体神经桥接修复后靶皮肤局部控温调节功能重建初步研究

  目的 探讨周围神经损伤修复后靶皮肤局部控温调节功能的重建.方法 本研究采用大鼠坐骨神经10 mm 缺损自体神经桥接修复模型,以神经缺损组作为对照,于术后即时和术后1、3、6个月,应用激光多普勒血流灌注成像仪动态分析足底皮肤血流灌注情况及其对冷刺激的反应,并采用红外热痛阈测定仪评定足底皮肤痛觉;于术后6个月时采用电生理检测、靶肌湿重比、神经与皮肤免疫组织化学染色等方法评价神经再生情况.结果 坐骨神经缺损后靶皮肤血流灌注骤然上升达基础水平的4.5倍左右,但冰水刺激并不引起灌注进一步增加;术后1个月,自体神经修复组和缺损组冰水刺激引起皮肤血流灌注改变量均较新鲜损伤时增加,但明显低于正常水平;术后3个月和6个月,自体神经修复组冰水刺激引起的灌注改变量与对侧正常皮肤相当,但缺损组则明显低于正常水平,这与痛觉测定结果一致;术后6个月时自体神经修复组神经干动作电位振幅恢复率、靶肌湿重比达75%左右,桥接段远侧神经纤维再生及靶皮肤再支配良好.结论 自体神经桥接修复大鼠坐骨神经缺损后靶皮肤局部控温调节功能得以较好重建.
