Benign intracranial cysts are not uncommonly identified as incidental findings on radiological imaging studies. They are generally asymptomatic and do not require surgical intervention. However, precise histological diagnosis and characterisation of these cysts may be challenging, even when biopsies have been undertaken. We report our experience in managing an adult patient with documented radiological and symptomatic enlargement of a right frontal neuroepithelial cyst treated with endoscopic fenestration.Despite adequate tissue sampling, histological diagnosis and characterisation of the cyst proved challenging. To our knowledge a case of this nature has not previously been reported.
