目的了解Astograph法支气管激发试验过程中不良反应发生情况及影响因素。方法选择937例患者用日本产支气管激发试验仪(CHEST公司的Astograph),由自动雾化发生装置、连续呼吸阻力测定装置和计算机显示(或描记)装置,受试者平静呼吸连续吸入由低到高浓度的激发剂的同时,仪器可自动而连续描记受试者的呼吸阻力(Rrs)的变化,结果由计算机处理后在屏幕上显示并可输出。显示或输出的结果包括呼吸阻力(Rrs)曲线等多项指标。观察在支气管激发试验过程中患者不良反应的发生情况及相关因素。结果 937例接受支气管激发试验患者中,年龄4~79岁,男性345例,女性592例,出现不良反应者268例(28.6%),其中气短、咳嗽、咽痒、咽干、胸闷、恶心、心慌等分别为171例(63.8%)、119例(44.4%)、82例(30.6%)、52例(19.4%)、37例(13.8%)、8例(3%)、7例(2.6%)其中激发试验阳性组气短症状发生率明显高于激发试验阴性组(分别为67.2%,33.0%;χ2=5.458,P=0.08)。性别、年龄、病程对不良反应发生率无影响,气道反应性越高气短症状越明显。结论 Astograph法支气管激发试验过程中,一过性不良反应发生率较高,其中咳嗽、咽痒、气短症状最常见。其中气短因素对试验影响最大,激发试验阴性者咳嗽症状发生率最高(48%),其次为咽痒(37%)、气短(33%)。937例接受支气管激发试验患者中,总不良反应发生率为28.6%,说明Astograph法支气管激发试验是安全有效的。
Objective To understand the incidence and influencing factors of adverse reactions in Astograph bronchial provocation test. Methods Nine hundred and ninety-seven patients were treated with a Japanese-made bronchial provocation test instrument (CHEST company Astograph). The device consisted of an automatic atomization device, a continuous respiratory resistance measurement device and a computer display (or tracing) device. At the same time, the instrument automatically and continuously traces the changes in the respiration resistance (Rrs) of the subject. The result is displayed on the screen and output by the computer. Display or output results include respiratory resistance (Rrs) curve and many other indicators. To observe the occurrence of adverse reactions in patients with bronchial provocation test and related factors. Results Among the 937 patients who underwent bronchial provocation test, there were 345 males and 592 females and 268 patients (28.6%) with adverse reactions. Among them, they were short of breath, cough, itchy throat, throat, chest tightness, nausea, (63.8%), 119 cases (44.4%), 82 cases (30.6%), 52 cases (19.4%), 37 cases (13.8%), 8 cases (3%) and 7 cases %) The incidence of shortness of breath symptoms in the positive test group was significantly higher than that in the negative test group (67.2% and 33.0%, respectively; χ2 = 5.458, P = 0.08). Sex, age, duration of adverse reactions had no effect on the incidence of airway reactivity, the more obvious symptoms of shortness of breath. Conclusion Astograph bronchial provocation test process, a higher incidence of adverse reactions, including cough, itchy throat, shortness of breath symptoms are most common. Among them, shortness of breath had the strongest impact on the test, while the negative test showed the highest incidence of cough (48%), followed by itchy throat (37%) and shortness of breath (33%). 937 cases of patients receiving bronchial provocation test, the total incidence of adverse reactions was 28.6%, indicating Astograph bronchial provocation test is safe and effective.
