低剂量131I治疗低刺激性Tg、不伴有远处转移的高危分化型甲状腺癌

目的:目前尚无低剂量131I用于治疗低刺激性Tg、不伴有远处转移的高危分化型甲状腺癌的相关研究,本研究旨在探讨低剂量131I在这部分人群中的清甲效果.方法:纳入本回顾性研究的标准为:1)年龄>18岁;2)分化型甲状腺癌全切术后;3)T4分期(术中肉眼可见甲外侵犯),任何N分期;4)清甲治疗前刺激性Tg≤5ng/ml;5)无远处转移的证据;6)TgAb≤46IU/ml.这部分患者在TSH刺激状态下(TSH≥30μIU/ml)分别随机接受了低剂量(30m Ci)和高剂量(100m Ci)放射性碘,之后予以TSH抑制治疗.治疗后6-10月在TSH刺激状态下检测血清Tg水平,并结合颈部B超及131I全身显像评价两组患者的清甲成功率.
