Objective: To test the reliability and validity of Beck hopelessness scale (BHS) in rural suicides and their controls, and explore the relationship between BHS and rural suicide.Methods: Data of 200 suicides happened from September 1, 2008 to August 31, 2009 were collected from 25 towns of 3 counties which were selected from disease surveillance points of Shandong Province.One informant of each suicide or paired control with same age (no more than 3 years), same gender and same district as suicide was interviewed by the trained interviewers.The Beck Hopelessness Scale (BHS) was used to test hopelessness in this study, with the Trait Anxiety Inventory (TAI) and the Barratt Impulsiveness Scale-11 (BIS-11) as the criterion.Results: The Cronbachs α of BHS in suicides and controls were 0.915 and 0.861, respectively.Criterion validity analysis demonstrated a positive correlation between the scores of BHS and BIS-11 and TAI (r=0.467, 0.718, P<0.001).Exploratory factor analyses were carried out in the suicidal and control groups, 4 factors in suicides and 5 factors in controls were yielded, which were different from the hypothesis of the scales original designer.Multifactor analysis showed that after adjusting the psychiatric disorder and marital status, the OR between hopelessness and suicide is 1.118.Conclusion: The Beck Hopelessness Scale (BHS) has good reliability and criterion validity, construct validity needs to be improved.Hopelessness might be a risk factor of complete suicide in rural population.
