Gastrointestinal symptoms are common.Depending on the symptoms profile, dyspeptic symptoms ranged from 15-40%, reflux symptoms 15-25% and irritable bowel symptoms 10-20%.A proportion of patients have an underlying organic cause for their symptoms such as peptic ulcer disease or reflux esophagitis.Most young patients will not have any organic disease assessed by an upper endoscopy."Test and treat" strategy for H.pylori infection is a part of consensus in patients under 45.The prevalence of H.pylori infection in a group 25-44 years of age is reported up to 38%.The European Maastricht Ⅳ Consensus recommended serology as an alternative option for countries with a high prevalence of H.pylori infection.In USA and Canada patients under 55 years of age and having dyspepsia without alarm symptoms should be evaluated for H.pylori infection.UBT, stool test (HPSA) and ELISA are recommended for non-invasive testing.Two serology tests are the most used: the laboratory ELISA test and rapid blood tests (Helicotest).The serology tests have the lowest cost per correct diagnosis at low (30%), intermediate (60%) and high prevalence (90%) of H.pylori infection but diagnostic accuracy is lower than other non-invasive tests.The UBT is more expensive, needs special preparation by the patient, such as fasting and cessation of antibiotics, proton pump inhibitors (PPI) and H2 blockers.The increasing antibody level on ELISA, assuming a positive result as an antibody level of 1.12 (U/ml) using the GLZ model (Generalized Linear Model) and 1.07 (U/ml) using the GAM model (Generalized Additive Model), showed the increasing probability of receiving a positive UBT.The Helicotest showed the highest prevalence of H.pylori infection but the mean IgG level by positive Helicotest and negative UBT was 0,86 U/ml.The kappa coefficient for study group was 0,92 for UBT and ELISA and 0,66 for UBT and Helicotest.Analysis of the data showed difference in detection of IgG level among patients withacute, moderate or mild dyspeptic symptoms.Patients with acute dyspeptic symptoms had the highest mean level of igG, 4.18 U/ml.The data confirms that there is a correlation between the level of IgG antibodies and positive UBT test.The level of IgG ELISA is a good predictor of probability of positive UBT test.Serological testing by HelicoTest showed the highest prevalence H.pylori infection in young dyspeptic patient.
