  object Cognitive instability is widely observed in several diseases and is amplified in conditions of sleep deprivation(SD).One of the gold standards for assessing individual cognitive instability in SD is assessment of lapses in the psychomotor vigilancetest(PVT).However,the number of PVT lapsesexhibits large interindividual differences that range from apparent cognitive resistance to severe cognitive impairment.In this study,we investigated whether the ability to sustain a moderate level of attention under conditions of SD was related todifferences in WM structure. method The participants were divided into resilient and vulnerable groups according to the mean number of PVT lapses in the final three hours of an SD session.Tract-based spatial statistical analyses with multiple diffusion tensor imaging-derived characteristics [i.e.,fractional anisotropy(FA),mean diffusivity (MD),radial diffusivity(RD),and axial diffusivity(AD)] were employed to investigate differences in white matter(WM)integrity between the resilient and vulnerable groups.At the group level,tract-based spatial statistics were also used to test the linear relationship between the number of PVT lapses and the diffusion characteristics across the WM skeleton. result Compared with the vulnerable group,the resistant group exhibited significantly increased FA in multiple brain regions including the corpus callosum(genu,body and splenium),right superior longitudinal fasciculus,right anterior corona radiata,bilateral superior corona radiata,bilateral posterior corona radiata,bilateral posterior thalamicradiata,left external capsule,left cingulum,right tapetum,left cerebral peduncle,and right posterior limb of the internal capsule.The whole WM skeleton correlation results revealed a large overlap with the group results,and the FAs in these regions exhibited significant negative correlations with the numbers of PVT lapse. conclusion Collectively,these findings suggested that cognitive instability after SD is likely a consequence of individual differences in WM integrity.

目的 探讨保留骨骺的骨搬运术在儿童四肢肿瘤保肢手术中的应用及早期疗效。方法 2010年3 月- 2014 年3 月,采用保留骨骺的骨搬运术治疗5例四肢肿瘤患儿,3 例骨肉瘤患儿,1例多发骨软骨瘤病,1例多发内生软骨瘤病。
