  Background: Extremely sharp angular spinal deformity of healed tuberculosis can be corrected by vertebral column resection (VCR).However,the VCR techniques have many limitations including spinal column instability,greater blood loss,and greater risk of neurologic deficit.Methods: To avoid these shortcomings,we have developed a new spinal osteotomy technique—Vertebral column decortication,including multilevel vertebral decancellation,removal of residual disc,osteoclasis of the concave cortex,compression of the convex cortex accompanied by posterior instrumentation with pedicle screws,with the expectation to decrease surgical-related complications.In this article,we describe a vertebral column decortication for thoracic kyphosis.Results: The patient underwent kyphosis correction and was corrected from 78 degrees to 34 degrees.The neurological function improved from JOA scale 3 to 7.The back pain and neurological function were significantly improved.The ODI decreased from 92 to 34.There was also a significant decrease in back pain VAS from 6 to 2.Conclusions: For cases with extremely severe Potts kyphotic deformity,the technology of mVCR offers excellent clinical and radiographic results.
