Background: Most highly lethal suicide attempters are admitted to emergency care units.Previous suicide attempt is known to be a strong predictive factor for suicide completion, and early crisis intervention including confirmation of suicide ideation is an important initial step for medical staffs to reduce the patients risk of later suicide reattempt.Objective: The goal of the present study is to examine the significance of early crisis intervention at emergency units by investigating feeling of highly lethal suicide attempters for the intervention.Methods: Study participants are all suicide attempters who were admitted to the emergency unit of Yokohama City University Medical Centre from April 1, 2010 to March 31, 2011.After recovering from impaired consciousness, they are immediately asked about 1) psychosocial background, 2) suicide ideation, 3) feeling of regret, 4) suicide method, and 5) feeling for communication on the present suicide attempt, by nurses, or psychiatric social workers at the emergency unit according to the structured interview form.The nurses and psychiatric social workers had been given lectures on suicide prevention and suicidal communications.Results: Among all suicide attempters, 82 attempters were admitted to the emergency unit, and 35 attempters (9 males and 26 females; mean age: 35.9±11.5) could answered the question.When asked about the suicide attempt, the majority of them felt confused (62.9%) and sad (54.3%).Those who felt uncomfortable (22.9%) and angry (14.3%) were fewer.Discussion: Fewer patients had negative feeling for intervention; early crisis intervention in emergency units was considered effective.On the other hand, we should further develop the method of intervention in order to increase effectiveness.After this early intervention, we introduced and continued psychiatric treatment and social work.We plan to exam the effectiveness of our intervention on preventing suicide reattempt.
