鼻中隔穿孔修补方法的探讨

  目的:探讨鼻中隔穿孔修补的方法,提高鼻中隔穿孔修补的成功率.方法:回顾性分析39例鼻中隔穿孔修补术患者的病例资料,根据鼻中隔穿孔原因,穿孔部位及面积大小选择不同鼻中隔穿孔修补方法.结果:39例患者(非医源性鼻中隔穿孔组12例,医源性鼻中隔穿孔组27例):其中穿孔面积<0.5cm14例,采用鼻中隔粘膜“+”字交叉对位缝合法修补,随访2年未再次穿孔;0.5cm<穿孔面积<1.5cm13例,采用鼻中隔粘膜交叉减张错位缝合法修补,随访1.5年未再次穿孔;穿孔面积>1.5cm7例,采用鼻底粘膜瓣错位修补法修补,随访1.5年2例发生再次穿孔;穿孔面积>2cm5例,采用自制带血管蒂粘骨膜瓣修补法修补,随访1.5年1例发生再次穿孔.

目的:报告1例特殊的先天性脑膜膨出合并脑脊液鼻漏的病例,分析探讨筛骨垂直板及唇腭发育的关系。方法:介绍1例罕见的鼻中隔内脑膜膨出,回顾脑膜膨出的类型及筛骨垂直板及唇腭的发育过程,讨论其可能发病机制,总结该病诊断及治疗的方法。结果:发现脑膜膨出可通过未完全闭合的筛骨垂直板发生于鼻中隔内并导致鼻中隔囊性肿物破裂后出现脑脊液鼻漏极易误诊,回顾性分析发现患者同时伴有先天性唇腭裂术后恢复不佳。
