  Aim: Impairment of intestinal motility and related infective complications is the predominant clinical phenomenon among most of the severe acute pancreatitis (SAP) patients.But western medicine effective in prevention or treatment of enteroparalysis in SAP is still lacking.We aim to investigate whether Da-Cheng-Qi decoction (DCQD) is beneficial to gastrointestinal injury in SAP;if it is,then to explore the mechanism involved.Method: DCQD was enema administered to 70 patients for 7 days.Mortality,(multi)organ failure during admission were observed,blood samples for laboratory analysis were drawn on admission,on days 3,7,and 14 of the treatment.We also experimentally examined plasma IL-1 β、 TNF-α and IL-10 assays in rat serum and carry the morphometric studies of the gut.Intestinal propulsion index,serum and tissue vasoactive intestinal peptide (VIP) were also detected.Result: Notable among these clinical results are although DCQD did not affect the overall incidence of patients with organ failure,it did shorten the average time that patients suffered from paralytic intestinal obstruction and decreased the infection incidence.Compared with untreated rats,the treated group showed significantly lower levels of proinflammatory cytokine IL-1β、TNF-α and lower mean pathological intestinal lesion scores.The VIP level in intestinal tissue or serum in DCQD group was obviously lowered and intestinal propulsion index was significantly improved.Conclusion: DCQD has good prospects in the treatment for SAP in the clinic.It exerts therapeutic effects through inhibiting the production of inflammatory mediators,mitigating the pathological damage of the intestinal injury,and increasing the intestinal propulsion index in SAP model.
