Positive muscle phenomena refer to spontaneous muscle hyperactivity, including spasms, tetany, fasciculations, myokymia, cramps, myotonia, rippling, percussion induced rapid contractions and myoedema.Experimental and clinical data indicate that positive muscle phenomena are common in a wide variety of neurological and non-neurological diseases.However, clinical and scientific data on these phenomena is limited and their presence sometimes overlooked.This gap in our knowledge hampers effective diagnostics and adequate treatment.In this presentation we will show examples of the different positive muscle phenomena and their role in diagnosing neuromuscular disorders.Clinical characteristics, diagnostic possibilities and associated disorders will be discussed.Finally, we will propose an algorithm in order to discriminate between the different muscle phenomena.
