PURPOSE To evaluate the feasibility of the monoexponential model of DWI,biexponential and stretched-exponential models of intravoxel incoherent motion(IVIM)in differentiating hepatocellular carcinoma(HCC)from hepatic hemangioma(HH)METHOD AND MATERIALS 44 patients were retrospectively enrolled from August 2015 to February 2016.With followed histopathological examination and clinical proof,24 lesions in 23 patients were confirmed as HCC,22 lesions in 21 patients were confirmed that HH.All the patients were performed MRI exams on an 1.5T scanner(GE-Signa HDXT,America)in a protocol containing the routine T1WI,T2WI,LAVA,DWI(b=0,600s/mm2),eDWI(b=0,20,50,100,150,200,400,800,1200,2000,3000s/mm2)and HB DWI(b=0,1500,2000,3000,4000s/mm2).For each ROI,the mean value of DWI(ADC),eDWI(D-mono,D*-mono,fmono,D-Bi、D*-Bi、fBi)and HB DWI(DDC,α)were measured.The ROI were drawn in the tumor parenchyma.ICC test was used to examine the consistency of the measurements between two observers.If the parameters fitted normal distribution,the students t-test was executed to compare the obtained parametric values,otherwise Mann-Whitney test was executed to compare the values.The ROC curves of all the parameters were drew and analyzed and the values concerned obvious difference(P < 0.05).RESULTS The ICC value in the HCC and HH group were exhibiting an amenable consistency.The ADC,D-mono,D*-mono,fmono,D-Bi,D*-Bi,fBi,DDC values of HCC and HH group remained statistically significant(P<0.05).The area under the ROC curve of ADC,D-mono,fmono,D-Bi were higher than 0.9.The area under the ROC curve of D*-mono was 0.845 with good diagnostic efficiency.The area under the ROC curve of D*-Bi,fBi,DDC was low.The diagnostic efficiency D-mono、D*-mono、fmono were higher than D-Bi,D*-Bi,fBi.CONCLUSION The D-mono value were significantly better than ADC in differential diagnosis of HCC and HH; The D*-mono,D*-Bi,fBi from biexponential models and DDC value from stretched-exponential model have certain ability in diagnostic differentiation of HCC and HH.The double monoexponential model(D-mono,D*-mono,fmono)was significantly better than the double biexponential model(D-Bi、D*-Bi、fBi)in differential diagnosis of HCC and HH.
