脑卒中后抑郁患者前额叶的氢质子磁共振波谱研究

目的:探讨卒中后抑郁障碍(post-stroke dpression,PSD)患者前额叶氢质子磁共振波谱(Proton magnetic resonance spectroscopy,1H-MRS)的变化特点. 方法:应用1H-MRS成像技术检测13例脑卒中后抑郁症患者(PSD组)和20例脑卒中后无抑郁患者(non-PSD组)前额叶N-乙酰天门冬氨酸(NAA)、胆碱(Cho)、肌酸(Cr)3种代谢物的峰下面积,计算NAA/Cr 和Cho/Cr比值.计量资料根据正态性检验结果,采用两样本t检验或Mann-Whitney U检验及双变量相关分析进行统计分析. 结果:与non-PSD 组比较,PSD 组双侧前额叶Cho/Cr比值显著升高,而左侧前额叶NAA/Cr比值显著下降,右侧前额叶NAA/Cr比值差异无统计学意义.在PSD组中,汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评分与左侧前额叶NAA/Cr比值呈显著正相关(P﹤0.05). 结论:PSD患者双侧前额叶的代谢物存在异常,提示其与PSD的发生可能存在密切的关系.同时,左侧前额叶NAA/Cr比值的大小对PSD患者抑郁严重程度的判断有一定的临床价值.
