In percutaneous coronary interventions (PCI), a complex lesion may be classified according to anatomic criteria, including extensive calcification, thrombus, and chronic occlusions, or by location, such as bifurcations, saphenous vein grafts (SVG) and unprotected left main.This presentation will focus on calcified, bifureation, and SVG lesions.CALCIFIED LESIONS: Fibrocalcific deposition enhances lesion rigidity, thereby complicating device delivery and deployment.Technical failure owing to inability to pass stents occurred more frequently in patients with calcified lesions than in those with noncalcified lesions, Balloon dilatation of heavily calcified lesions can increase risk of dissection or perforation.
