Background: Acute Myocardial Infarction (AMI) serious threat to human life and health, Cardiac Rupture (CR) is one of the most serious complications in AMI, CR pathogenesis and effective treatment methods 1s not clear yet and need further study.Objective: To analyze the characteristics, early diagnosis and the suitable therapy of cardiac rupture (CR) in patients with acute myocardial infarction (AMI).Methods: We enrolled consecutive 1526 patients with AMI from March 2000 to March 2010 in our hospital.Through echocardiography and pericardiocentesis, we found that patients with CR (CR group) were 30 cases, patients with no CR were 1496.Combined with the results of coronary angiography (CAG), we explored the risk factors and effective treatment of CR after AMI.Results: Compared to patients with non-cardiac rupture, elderly patients, female gender were significantly higher in CR group.Patiens with anterior infarction had higher incidence of CR, especially in ST elevated infarction.Patients who accompanied with hypertension, diabetic mellitus, abnormal renal function were more likely to occur CR.Patients who accompanied with enlarged LVEDD, EF less than 50% and LAD lesion were more likely to occur CR, especially ostium lesion of LAD and LAD accompanied three vessel lesions.Patients with successful reperfusion were significantly less likely to occur CR (P<0.05).Conclusion: The prognosis of CR after AMI was poor.Patients who were female, older and who were with anterior infarction or severe LAD lesion were more likely to occur CR.Early prevention and early success of reperfusion therapy can reduce the occurrence of CR.
