动态增强MRI联合扩散加权成像评价脑外伤后高压氧治疗疗效的实验研究

目的:利用DCE-MRI和DWI来动态观测HBO治疗后脑外伤组织本身以及周围区域的变化情况,为HBO的临床应用提供影像学基础。方法:42只实验兔随机分为4组,即外伤(TBI)组(n=15),外伤+高压氧(TBI+HBO)组(n=15),假手术(Sham)组(n=6),假手术+高压氧(Sham+HBO)组(n=6).TBI组和TBI+HBO组接受自由落体打击以造成脑外伤,而Sham组和Sham+HBO组只进行开颅手术。TBI+HBO组和Sham+HBO组接受7d共10次的高压氧治疗,而TBI组和Sham组不进行高压氧治疗。结论:本研究结果表明HBO能够缩短局部病灶区血脑屏障开放时间,降低开放幅度,推迟细胞毒性水肿的发生,降低细胞毒性脑水肿的程度,加速病灶周围区血脑屏障的修复,延缓周围区细胞损伤的发生,并且在亚急性期减轻损伤的程度,进而减少了局部病灶区和周围区神经元的丢失,促进TBI兔神经功能的恢复。
