目的：观察和评价外配式耳甲迷走神经刺激仪治疗原发性失眠障碍的临床疗效.方法：对符合《美国精神疾病分类和诊断标准》第5 版(DSM-5TM)有关原发性失眠症诊断标准的35 例患者,于耳甲腔迷走神经分布区使用自主研发的华佗牌外配式耳迷走神经刺激仪(TENS-200A 型),每日治疗一次,每次30 分钟,持续进行4 周治疗,6 周随访.通过匹茨堡睡眠质量指数(PSQI)、14 项汉密顿焦虑量表(HAMA)、17 项汉密顿抑郁量表(HAMD)进行评分,采用自身前后对照法,观察入组前(第0 周)、治疗中第1、2、3、4 周以及第6 周随访时各量表的评分,对比治疗前后的评分变化.结果：与基线相比较,治疗4 周后患者PSQI 总分下降26.1％,第6 周随访时下降达29.1％；HAMA 总分在治疗4 周后下降43.4％,第6 周随访时可下降52.6％；HAMD 总分在治疗4 周后下降36.7％,第6 周下降可达47.3％.结论：耳迷走神经刺激仪可缓解睡眠障碍症状,并能同时改善原发性失眠患者的焦虑、抑郁等情感障碍,具有一定的远期疗效.
