目的 探讨急性白血病 (AL)原代细胞survivin基因表达及其与AL疗效的关系。方法 应用RT PCR方法检测 50例初发AL患者survivin基因mRNA表达。结果 AL细胞survivinmRNA阳性率为 82 .0 % (50例中 41例 )。survivinmRNA阳性表达在急性淋巴细胞白血病细胞 (89.5 % )较在急性非淋巴细胞白血病 (ANLL)细胞 (75 .0 % )稍多见 ,但均高于正常对照组 (33 .3 % ) ,P值分别 <0 .0 1 ,<0 0 5。在 2 2例ANLL患者白血病细胞中 ,survivinmRNA表达阴性者接受 1个疗程化疗后骨髓缓解(BMR)率 (83 .3 % )明显高于阳性者 (2 5 .0 % ,P =0 .0 2 3) ;1 3例ANLL患者接受高三尖杉酯碱和阿糖胞苷联合治疗 ,survivinmRNA阴性表达者BMR率 (1 0 0 .0 % )高于阳性表达者 (2 7.3 % ) ;survivin/ β actin >0 .6者BMR率低。结论 survivin基因在AL细胞高表达 ,可能是导致AL细胞对化疗药物不敏感的原因之一
Objective To investigate the expression of survivin gene in acute leukemia (AL) primary cells and its relationship with the efficacy of AL. Methods RT - PCR was used to detect the expression of survivin mRNA in 50 cases of primary AL. Results The positive rate of survivin mRNA in AL cells was 82.0% (41 of 50 cases). The positive expression of survivin mRNA was more common in acute lymphoblastic leukemia cells (89.5%) than in acute non-lymphocytic leukemia (75.0%) patients, but both were higher than those in normal control group (33.3%), P value Respectively <0. 01, <0 0 5. In 22 ANLL patients with leukemia cells, the bone marrow remission (BMR) rate (83.3%) was significantly higher than that of the positive ones (25.0%, P = 0.002) after one course of chemotherapy and negative survivin mRNA expression 3). The BMR rate (100.0%) of survivin mRNA negative expression was higher than that of positive expression (13.3%) in 13 ANLL patients treated with combination of homoharringtonine and cytarabine. Survivin / β Actin> 0.6 BMR rate is low. Conclusion The high expression of survivin gene in AL cells may be one of the reasons that AL cells are not sensitive to chemotherapy drugs
