As donor and recipient criteria for pulmonary transplantation have become more liberal, the anesthetic management of these patients has become more challenging.This presentation will review the indications and medical assessment for patients presenting for pulmonary transplantation, the standard and extended criteria for pulmonary donation, and the perioperative management including the preoperative assessment, the intraoperative management including the use of transesophageal echocardiography, ventilation strategies to prevent post-transplant pulmonary injury, pharmacologic strategies to support cardiac output during offpump pulmonary transplantation, and the indications and initiation of pulmonary mechanical assistance, including the use of and timing for extracorporeal membrane oxygenation, and postoperative care including ventilation strategies and pain control.The anesthetic management of patients with previous pulmonary transplantation will also be discussed.
