心力衰竭患者心房κ-阿片肽受体表达的研究

背景:心力衰竭(CHF)是老年人最常见的疾病之一,是日益严重的公众健康问题。人们早已知道内源性阿片肽(EOP)的存在,但以往的研究多集中在中枢水平。随着研究的进展,发现除中枢外,心肌细胞膜和血管壁存在大量以阿片肽κ受体为主的阿片肽受体系统,心脏亦可合成EOP,提示阿片肽对心血管系统具有自分泌调节作用。心血管EOP系统与阿片受体可能在心脏疾病的发生、发展过程中有一定的意义。目前受体及其信号通路研究已经成为心力衰竭研究的重要方向。而心力衰竭的发生与心脏结构、功能和电生理特性的变化即所谓的心脏重塑有关。近来阿片受体与心力衰竭心脏重塑相关研究逐渐增多,但结论仍有争议。因此探明阿片肽受体与心力衰竭之间的关系将进一步增强对于心力衰竭发病机制的认识。目的:探讨心力衰竭(CHF)对心房κ-阿片肽受mRNA及蛋白表达的影响。方法:我们在24例心功能正常及24例心力衰竭(CHF)患者的心房组织中,采用RT-PCR、免疫组织化学及电镜方法,从基因水平、蛋白水平及形态学依据三个方面研究其心房κ-阿片肽受体mRNA及蛋白表达的差异。结果:与心功能正常组相比,心力衰竭组κ-阿片肽受体mRNA及蛋白表达量增强。心力衰竭组患者的线粒体平均形态增大。结论:在心力衰竭时,κ-阿片肽受体及其前体POMC mRNA及表达量增强,心房组织阿片肽系统蛋白表达量呈显著上调上调,表明心力衰竭时的心房组织自我保护能力增强。心力衰竭组患者的线粒体平均形态增大,可能是由于阿片肽系统蛋白表达量增高而引起的心房肌细胞超微结构重构的表现。
