目的通过对急性一氧化碳中毒及其迟发性脑病患者外周血及脑脊液的多巴胺(DA)和乙酰胆碱(Ach)DA、Ach水平的对照研究,探讨DA和Ach在急性一氧化碳中毒后迟发性脑病发病机制中的作用。方法分别用荧光分光光度法及Hestrin碱性羟胺比色法对14例急性一氧化碳中毒患者、34例迟发性脑病患者(急性期及恢复期)和21例正常人的外周血和脑脊液进行TDA、Ach水平的检测,测定结果进行了方差分析。结果迟发性脑病患者急性期血清及脑脊液DA水平显著降低,恢复期血清中DA水平与对照组比较无显著差异,脑脊液中DA水平仍低于正常。迟发性脑病组急性期全血Ach显著降低,脑脊液中Ach明显增高,恢复期恢复正常。急性一氧化碳中毒患者全血Ach水平明显高于迟发性脑病组及正常组,但血清DA与正常组、迟发性脑病组无显著性差异。结论DA、Ach水平异常在急性一氧化碳中毒后迟发性脑病的发生中起一定作用,并与迟发性脑病的预后有关。
Objective To investigate the relationship between DA and Ach in the pathogenesis of delayed encephalopathy after acute carbon monoxide poisoning by comparing the levels of dopamine (DA) and acetylcholine (Ach) DA and Ach in peripheral blood and cerebrospinal fluid in patients with acute carbon monoxide poisoning and its delayed encephalopathy In the role. Methods The peripheral blood and cerebrospinal fluid of 14 patients with acute carbon monoxide poisoning, 34 patients with delayed encephalopathy (acute stage and convalescent stage) and 21 normal subjects were analyzed by fluorescence spectrophotometry and Hestrin basic hydroxylamine colorimetry, respectively. TDA, Ach level of detection, measurement results were analyzed by ANOVA. Results Serum and cerebrospinal fluid levels of DA in patients with delayed encephalopathy were significantly decreased. There was no significant difference in serum DA levels between convalescent patients and control subjects. The level of DA in cerebrospinal fluid was still lower than normal. The Ach of whole blood in acute encephalopathy group was significantly decreased, Ach in cerebrospinal fluid was significantly increased, recovery time returned to normal. Ach patients with acute carbon monoxide poisoning were significantly higher than those in patients with delayed encephalopathy and normal group, but serum DA and normal group, delayed encephalopathy no significant difference. Conclusion Abnormal levels of DA and Ach play a role in the pathogenesis of delayed encephalopathy after acute carbon monoxide poisoning and are associated with the prognosis of delayed encephalopathy.
