目的:探讨神经导航结合术中超声检查在幕上胶质瘤外科治疗的应用价值。方法:对31例幕上胶质瘤患者根据导航系统引导开颅,术中根据超声成像判断肿瘤大小及边界,制定最佳手术方案,显微切除肿瘤,术中实时超声判断肿瘤切除程度。通过术后病理学结果及影像学复查对该方法进行分析。结果:肿瘤全切29例,大部切除2例。所有患者导航引导开颅范围精确;术中超声成像定位精确,不同级别胶质瘤超声表现有差异。术后2例出现轻至中度神经功能障碍,无一例发生感染。结论:神经导航结合术中超声检查可协助对幕上胶质瘤行准确有效的外科治疗。
Objective: To explore the value of neural navigation combined with intraoperative ultrasound in the surgical treatment of supratentorial glioma. Methods: Thirty-one cases of patients with supratentorial glioma underwent craniotomy guided by the navigation system. During the operation, the size and the boundary of the tumor were determined according to the ultrasound imaging. The best surgical plan was made and the tumors were removed by microsurgical excision. The method was analyzed by postoperative pathology and imaging review. Results: Tumor resection in 29 cases, most of the resection in 2 cases. All patients guided navigation craniotomy accurate range; intraoperative ultrasound imaging precise positioning, different grades of gliomas have different ultrasound performance. There were 2 cases of mild to moderate neurological dysfunction after operation, and none of them developed infection. Conclusion: Neuro-navigation combined with intraoperative ultrasound can help accurate and effective surgical treatment of supratentorial gliomas.
