病人和方法对全组61例女性病人进行彩色多普勒血流显象,其中乳腺癌28例,乳房纤维腺瘤33例.乳腺癌病人平均年龄54岁,病灶直径8~37mm;乳房纤维腺瘤病人平均年龄38岁,病灶直径5~31mm.结果28例乳腺癌中的21例有血流显像,而在33例乳房纤维腺瘤中仅11例有血流显像,且只有当病灶大于13mm时才出现血流显像.由此可见,乳房恶性肿瘤的血流显像比例明显高于良性病变(P<0.01).乳腺癌病人的动脉收缩期血流速率的均值为1.0kHz,而乳房纤维腺瘤病人动脉收缩期血流速率的均值为0.5kHz.尽管良、恶性病变之间在动脉血流速率上有很大重叠,但5例动脉血流速率超过1.25kHz的均为恶性病变.7例病灶直径小于13mm乳腺癌中有5例监测到血流,且血管数量及动脉收缩期血流速率与病灶直径成正比,而8例直径小于13mm的乳房纤维腺瘤均未出现血流显像.对17例乳腺癌病人进行腋窝扫描,其中5例提示淋巴结周围有血流,但病理检查仅证实3例有腋窝淋巴结转移.12
