目的探讨中心静脉压(CVP)监测下,采用加压袋装液体快速扩容法抢救失血性休克的临床运用及效果。方法40例失血性休克患者随机分为研究组(Ⅰ组n=20)和对照组(Ⅱ组n=20),Ⅰ组在CVP监测下,采用血压计袖带加压软性袋装液体快速静脉输注法;Ⅱ组用传统输液方式处理。结果Ⅰ组病例在30m in内平均输入量为(1271±170)m l,在围术期平均总输入量为(5311±410)m l;Ⅱ组分别为(721±140)m l与(3273±330)m lP<0.01,SBP、DBP、CVP、SpO2指标恢复,排尿恢复时间,治愈率明显优于Ⅱ组;死亡率下降。结论在CVP监测下,实施加压快速扩容法是救治失血性休克的重要措施,可迅速恢复病人有效循环血量,维持重要器官组织细胞的灌注,提高抢救成功率。
Objective To investigate the clinical application and effect of rapid compression of bagged liquid to rescue hemorrhagic shock under central venous pressure (CVP) monitoring. Methods Forty patients with hemorrhagic shock were randomly divided into the study group (n = 20 in group Ⅰ) and the control group (n = 20 in group Ⅱ). In group Ⅰ, under the CVP monitoring, the sphygmomanometer cuff pressure soft bagged liquid Intravenous infusion; Ⅱ group with the traditional method of infusion. Results The average input volume of group Ⅰ was (1271 ± 170) ml within 30 mins, and the average total input during the perioperative period was (5311 ± 410) ml. The average input of group Ⅱ was (721 ± 140) ml and (3273 ± 330) ) m lP <0.01, SBP, DBP, CVP, SpO2 index recovery, urination recovery time, cure rate was significantly better than Ⅱ group; mortality decreased. Conclusion Under the CVP monitoring, the rapid pressurization expansion method is an important measure to hemorrhagic shock. It can quickly restore the effective circulating blood volume, maintain the perfusion of vital organs and improve the rescue success rate.
