目的探讨先天性心脏病(CHD)患儿心功能早期变化与血清miR-30a水平的相关性。方法选择新疆医科大学第一附属医院确诊的53例CHD患儿作为观察组,按不同心功能水平分为A组21例(无心力衰竭),B组32例(轻度心力衰竭),同期随机选取该院健康体检儿童50人为对照组。采用荧光定量PCR法测定血清miR-30a水平,组织多普勒成像(TDI)检测心功能指数,包括左室射血分数(LVEF)、右室射血分数(RVEF)、左室短轴缩短率(LVFS)、TDI-Z指数、TDI-Tei指数。结果三组间RVEF、miR-30R两两比较差异均有统计学意义;观察组除左心Tei指数外,其他指标组间比较差异均有统计学意义;血清miR-30a与左、右心TDI-Z指数呈负相关,血清miR-30a与左、右心TDI-Tei指数呈正相关。结论血清miR-30a参与了CHD心功能变化的过程,其水平随心功能程度波动,或可作为判断CHD心功能的监测指标。
Objective To explore the correlation between early changes of cardiac function and serum miR-30a levels in children with congenital heart disease (CHD). Methods Fifty-three children with CHD diagnosed at the First Affiliated Hospital of Xinjiang Medical University were selected as the observation group and divided into two groups according to different cardiac function levels: 21 cases in group A (no heart failure), 32 cases in group B (mild heart failure) Select the hospital health examination of 50 children as a control group. Serum levels of miR-30a were measured by real-time quantitative PCR, and cardiac function index including left ventricular ejection fraction (LVEF), right ventricular ejection fraction (RVEF), and left ventricular fractional shortening (LVFS), TDI-Z index, TDI-Tei index. Results There were significant differences in RVEF and miR-30R between the three groups in any pairwise comparison. In the observation group, except for the left ventricular Tei index, there were significant differences among the other index groups. Serum miR-30a and left and right heart TDI -Z index was negatively correlated, serum miR-30a and left and right heart TDI-Tei index was positively correlated. Conclusions Serum miR-30a is involved in the process of cardiac function changes in CHD. The level of miR-30a may fluctuate with the degree of cardiac function, or may be used as a monitoring index to determine the cardiac function of CHD.
