目的:探讨兔上颌窦外侧壁截骨术对上颌窦黏膜功能及MUC5AC表达的影响。方法:取新西兰成年兔12只,每只兔左侧为实验侧,全麻后切开龈颊沟黏膜,充分剥离,显露上颌骨前、外侧壁。上颌窦外侧壁截骨线做20 mm×2 mm上颌窦骨壁及黏膜缺损。右侧上颌窦不做任何处理作为对照侧。实验动物随机分为3组,每组4只,分别在术后2、4和8周处死一组动物。通过H-E染色和免疫组织化学染色观察上颌窦黏膜的组织学变化过程及杯状细胞的分泌情况。采用Graphpad Prism v5.0软件包对数据进行统计学处理。结果:术后2周,上颌窦黏膜附着纤毛脱落,且有腺体部分破坏、减少;术后4周,黏膜细胞水肿呈空泡样,黏膜固有层腺体、血管增生;术后8周,黏膜上皮层基本恢复正常,纤毛上皮减少、排列不完整,形态仍有异常。免疫组织化学染色发现,实验侧2、4、8周MUC5AC表达较对照侧显著增加(P<0.001)。2周时MUC5AC表达最强,4周时表达稍减弱,8周时表达明显减弱。结论:兔上颌窦外侧壁截骨术后的上颌窦黏膜分泌功能发生明显变化。随着时间推移,其组织结构在一定程度上可以自行修复,但与正常结构相比仍有一定差异。减轻手术对上颌窦及黏膜的损伤,可降低术后对其生理功能的影响。
Objective: To investigate the effect of rabbit lateral maxillary sinus osseous surgery on mucosal function and expression of MUC5AC in maxillary sinus. Methods: Twelve adult New Zealand rabbits were selected. The left side of each rabbit was the experimental side. The gingival buccal mucosa was excised after general anesthesia and the anterior and lateral maxillary anterior maxilla were dissected. Maxillary sinus lateral osseous osteotomy line 20 mm × 2 mm maxillary sinus bone and mucosal defects. The right maxillary sinus did not do any treatment as the control side. The experimental animals were randomly divided into 3 groups, 4 in each group, and animals were sacrificed at 2, 4 and 8 weeks after surgery respectively. Histological changes and goblet cell secretions were observed by H-E staining and immunohistochemistry. Data was statistically processed using Graphpad Prism v5.0 software package. Results: After 2 weeks, the mucosa attached to the maxillary sinus mucosa was exfoliated and some of the glands were destroyed and decreased. At 4 weeks after operation, mucosal cell edema was vacuolized and the lamina propria glands and vascular hyperplasia were found. After 8 weeks, Mucosal epithelial layer returned to normal, reduced ciliated epithelium, arranged in an incomplete form is still abnormal. Immunohistochemical staining revealed that the expression of MUC5AC on the experimental side was significantly higher than that on the control side at 2, 4, 8 weeks (P <0.001). MUC5AC expression was the strongest at 2 weeks, slightly weakened at 4 weeks, and significantly decreased at 8 weeks. Conclusion: The secretion of maxillary sinus mucosa in rabbit with maxillary sinus lateral wall osteotomy has obvious changes. With the passage of time, the organizational structure can be repaired to some extent by itself, but there are still some differences compared with the normal structure. Reduce the operation of the maxillary sinus and mucosal injury, can reduce the postoperative impact on their physiological function.
