[目的]检测溃疡性结肠炎(ulcerative colitis,UC)患者及健康人群血清25(OH)D3及跟骨骨密度水平,分析两者与UC临床类型和临床分期的关系,探讨其在疾病发生发展中的临床意义。[方法]纳入108例溃疡性结肠炎患者和96例健康对照者为研究对象,采用ELISA法检测溃疡性结肠炎患者和健康体检者血清中25(OH)D3水平,应用Mayo评分法对UC患者的疾病活动度进行分级,将25(OH)D_3水平与UC患者临床资料以及疾病活动度进行相关性分析;美国HOLOGIC公司生产的双能射线骨密度仪测量2组跟骨骨密度,并分析与UC的相关性。[结果]UC患者血清25(OH)D_3水平(10.64±5.72)ng/ml,显著低于对照组(13.41±6.27)ng/ml(P<0.05)。UC缓解期、轻度、中度和重度UC患者血清25(OH)D3水平分别为(12.84±3.28)ng/ml、(12.56±3.61)ng/ml、(8.96±3.15)ng/ml、(6.88±3.28)ng/ml。重度UC患者血清25(OH)D3水平均明显低于UC缓解期。UC患者跟骨骨密度(0.82±0.32)g/m2、T值:(-0.62±1.86),显著低于对照组(0.98±0.21)g/m2、T值:(-0.98±0.92)(P<0.05)。UC患者骨量减少和骨质疏松的发病率分别为37.9%和4.6%。[结论]UC患者存在血清25(OH)D3水平缺乏及骨量减少/骨质疏松且与病情严重程度相关。
