目的对在筛选结直肠癌的临床工作中应用粪便隐血化学检测与免疫检测的价值进行分析探讨。方法将本院在2014-2016年收治的高度怀疑为大肠肿瘤的患者共计200例作为研究资料,患者的平均年龄是(58.7±3.6)岁。行化学法检测的患者有100例,行免疫法检测的患者有100例,之后行镜检以统计分析应用免疫法和化学法的敏感性与特异性。结果在实施粪便潜血化学法检测的粪便标本里,有12例患者检查结果为阳性,阳性率为12.00%,其中,有10例患者为癌症患者,真阳性率为2.00%,假阴性率为8.0%,真阴性率为80.0%(80例),假阳性率为10.0%(10例),其敏感性为35.00%,95%的置信区间是19%-55%,特异性是90.00%,95%的置信区间是90%-93%。在应用粪便隐血免疫法检测的粪便标本里,有18例为结果阳性的患者,阳性率为18%,应用肠镜检测手段诊断为是肠癌的患者共16例,其中,真阳性率为10.00%,假阴性率为4.00%,真阴性率为78%,假阳性率为8.00%,敏感性是69.00%,95%的置信区间是52%-83%,其特异性为92%,95%的置信区间是92%-95%。结论在肿瘤患者的初步筛查中应用粪便化学法进行检测,免疫法是对那些存在高危患癌因素的人群较好的检测方法。
Objective To evaluate the value of chemical and fecal occult blood tests in screening clinical colorectal cancer. Methods A total of 200 patients with highly suspected colorectal cancer who were treated in our hospital from 2014 to 2016 were included in this study. The mean age of patients was 58.7 ± 3.6 years. There were 100 patients in the chemiluminescence assay and 100 in the immunoassay, followed by microscopic examination for statistical analysis of the sensitivity and specificity of the immunological and chemotherapeutic methods. In the fecal samples detected by fecal occult blood test, 18 patients were positive, with a positive rate of 18%. Of the 16 patients diagnosed as colon cancer by colonoscopy, the true positive rate was 10.00 %, The false negative rate was 4.00%, the true negative rate was 78%, the false positive rate was 8.00%, the sensitivity was 69.00%, the 95% confidence interval was 52% -83%, the specificity was 92% and 95% The confidence interval is 92% -95%. Conclusions The stool chemistry method is used in the initial screening of tumor patients. Immunoassay is a good method for detecting those with high-risk cancer.
