目的 评价三倍量 ( 0 .3mmol/kg)国产Gd DTPA增强MR探查脑膜转移瘤的效果。材料与方法 用国产Gd DTPA三倍量增强MR检查 110例临床怀疑脑转移瘤患者。先行平扫 ,然后静脉注射常规量 ( 0 .1mmol/kg)国产Gd DTPA。注射后即刻行T1WI扫描 ,2 0分钟后行延迟扫描。距第 1次注射 3 0分钟后 ,再注射 0 .2mmol/kg的Gd DTPA ,从而达到累积剂量 0 .3mmol/kg。第 2次注射后即刻及 2 0分钟后分别行T1WI扫描。对MR扫描所见进行定性、定量分析。结果 共有 70例患者确诊为有颅内转移 ,3 8例有脑膜转移 ,共发现 70个脑膜异常强化病灶。这 3 8例患者三倍量增强共多发现了 3 3个脑膜异常强化病灶 ,有 2 0例 ( 2 8.6% )三倍量增强扫描发现了更多的脑膜异常强化病灶 ,有 13例 ( 18.6% )疾病的定性诊断发生了变化。常规量增强和三倍量增强病灶或病灶部位平均相对信噪比 (C/N)分别为 6.10和 11.3 7,有显著性差异 (P <0 .0 5 )。结论 国产Gd DTPA三倍量 ( 0 .3mmol/kg)增强可提高脑膜转移瘤病灶的检出率。
Objective To evaluate the effect of three-fold (0.3 mmol / kg) domestic Gd DTPA enhanced MR detection of meningeal metastases. MATERIALS AND METHODS: 110 patients with suspected brain metastases were treated with triple Gd-DTPA enhanced Gd-DTPA. First plain scan, and then intravenous conventional amount (0 .1mmol / kg) of domestic Gd DTPA. T1WI scan immediately after injection, delayed scan after 20 minutes. After 30 minutes from the first injection, 0.2 mg / kg of Gd DTPA was injected again to achieve a cumulative dose of 0.3 mmol / kg. T1WI scan was performed immediately after the second injection and after 20 minutes. See the MR scan qualitative and quantitative analysis. Results A total of 70 patients were diagnosed as intracranial metastases, 38 cases of meningeal metastasis, found a total of 70 meningeal abnormal enhancement lesions. Thirty eight cases of patients with triple enhancement showed a total of more than 33 enhanced meningeal lesions, 20 cases (28.6%) three-fold enhancement scan found more enhanced meningeal lesions, 13 cases (18.6 %) The qualitative diagnosis of the disease has changed. The average relative signal-to-noise ratio (C / N) of conventional and triple-fold enhancement lesions or lesion sites were 6.10 and 11.37, respectively, with a significant difference (P <0.05). Conclusion Three-fold increase of domestic Gd DTPA (0.3 mmol / kg) can increase the detection rate of meningioma lesions.
