目的探讨重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并高血压患者血清肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)及乳酸脱氢酶(LDH)的变化,对比分析OSAHS患者心脏结构和功能异常率。方法选取2014年1月至2015年6月经多导睡眠监测确诊为重度OSAHS的患者103例,其中55例为单纯OSAHS组,48例为OSAHS合并高血压组。同时选取20例原发性高血压患者为单纯高血压组,20例健康查体人员为对照组。测定4组人群CK、CK-MB及LDH水平,并应用心脏彩超评价两组OSAHS患者心脏结构和功能。结果单纯OSAHS组、OSAHS合并高血压组的CK、CK-MB及LDH水平较对照组明显升高(P均<0.05);与单纯OSAHS组相比,合并高血压组的CK、LDH水平明显升高(P均<0.05),但CK-MB水平差异无统计学意义(P>0.05)。OSAHS合并高血压组左心室向心性肥厚、左心房增大、主动脉瓣返流、E峰与A峰比值(E/A)<1的发生率均明显高于单纯OSAHS组(P<0.05或P<0.01)。结论重度OSAHS患者的心脏结构和功能有一定的异常率,合并高血压者异常率会明显增加。
Objective To investigate the changes of serum creatine kinase (CK), creatine kinase MB (CK-MB) and lactate dehydrogenase (LDH) in patients with severe obstructive sleep apnea-hypopnea syndrome (OSAHS) Analysis of cardiac structural and functional abnormalities in patients with OSAHS. Methods A total of 103 patients with severe OSAHS diagnosed by polysomnography from January 2014 to June 2015 were selected, of whom 55 were OSAHS alone and 48 were OSAHS with hypertension. Twenty patients with essential hypertension were selected as hypertension group and 20 healthy individuals as control group. The levels of CK, CK-MB and LDH in four groups of people were measured. Cardiac color Doppler was used to evaluate the cardiac structure and function in two groups of OSAHS patients. Results The levels of CK, CK-MB and LDH in OSAHS group and OSAHS group were significantly higher than those in control group (all P <0.05). Compared with OSAHS group, the levels of CK and LDH in OSAHS group were significantly increased (P <0.05), but there was no significant difference in CK-MB level (P> 0.05). The incidence of left ventricular concentric hypertrophy, left atrium enlargement, aortic regurgitation, E / A ratio <1 in OSAHS patients with hypertension group were significantly higher than those in OSAHS group (P <0.05 or P <0.01). Conclusions There is a certain abnormal rate of cardiac structure and function in patients with severe OSAHS. The abnormal rate of hypertension with hypertension will be significantly increased.
