目的探讨覆盖全口义齿修复的临床效果。方法通过覆盖全口义齿与普通全口义齿的比较观察在有无牙根保留的情况下,牙槽骨的吸收状况。通过问卷调查纵向观测病例,考察义齿修复后各项使用功能(固位和稳定功能;咀嚼功能;语音;外观;舒适度;耐用性)的评价。结果牙槽骨吸收情况:覆盖全口义齿组:3个月前牙区(0.47±0.023)mm,后牙区(0.83±0.032)mm;6个月前牙区(0.63±0.028)mm,后牙区(1.02±0.031)mm;1年前牙区(1.11±0.026)mm,后牙区(1.67±0.035)mm;②普通全口义齿组:3个月前牙区(1.02±0.027)mm,后牙区(1.12±0.033)mm;6个月前牙区(1.53±0.029)mm,后牙区(1.83±0.036)mm;1年前牙区(1.99±0.026)mm,后牙区(2.25±0.024)mm。覆盖全口义齿组各项指标满意率分别为100%,70.1%,89.2%,100%,82.3%,77.3%。普通全口义齿组各项指标满意率分别为100%,66.2%,88.5%,100%,66.4%,88.9%。结论覆盖全口义齿修复后牙槽骨吸收较普通全口义齿明显缓慢。覆盖全口义齿和普通全口义齿都能满足固位和美观的要求。在咀嚼功能的恢复及舒适性方面,覆盖全口义齿优于普通全口义齿,而普通全口义齿较覆盖全口义齿耐用。
Objective To investigate the clinical effect of complete denture restoration. Methods The condition of alveolar bone absorption was observed by comparing the coverage of complete denture with that of general denture. Longitudinal observation cases were surveyed by questionnaire to evaluate the use of various functions (retention and stabilization function; masticatory function; speech; appearance; comfort; durability) after denture repair. Results Alveolar bone resorption: The total denture group was covered at 3 months with 0.47 ± 0.023 mm in the posterior teeth and 0.83 ± 0.032 mm in the posterior teeth, 0.63 ± 0.028 mm in the teeth 6 months earlier, (1.02 ± 0.031) mm in tooth area and 1.11 ± 0.026 mm in a year ago and 1.67 ± 0.035 mm in posterior tooth area respectively. (1.12 ± 0.033) mm in the posterior teeth area (1.12 ± 0.033) mm, the posterior teeth area (1.53 ± 0.029) mm and the posterior teeth area (1.83 ± 0.036) mm in 6 months, 2.25 ± 0.024) mm. The satisfaction rate of each index covering the complete denture group was 100%, 70.1%, 89.2%, 100%, 82.3% and 77.3% respectively. Satisfaction rate of each index of general complete denture group was 100%, 66.2%, 88.5%, 100%, 66.4%, 88.9% respectively. Conclusion The coverage of alveolar bone covered by the complete denture was obviously slower than that of the general denture. Cover full denture and general denture can meet the retention and aesthetic requirements. In terms of the recovery and comfort of chewing function, the complete denture is superior to the ordinary complete denture while the ordinary complete denture is more durable than the full denture.
