目的重视对胃黏膜下血肿的临床认识,提高临床治疗水平。方法对610份胃十二指肠溃疡引起上消化道出血胃镜存盘图片资料重新阅读,分析,找出18例胃黏膜下小血肿与2例确诊胃黏膜下血肿及结合临床病例资料,进行相关病因回顾性分析。结果胃黏膜下血肿多伴有应激性的生活事件,及胃、十二指肠溃疡等原发病。故胃黏膜下血肿可能是胃、十二指肠溃疡并发症之一,并多以小血肿(表面直径小于1cm)出现。讨论胃黏膜下血肿是胃、十二指肠溃疡应激状态下出现并发症之一。因血肿小,使临床不典型,不易辨认或遗漏观察。应激性生活事件是诱使胃、十二指肠溃疡伴出血及胃黏膜下血肿的最直接原因之一。
Objective To attach importance to the clinical understanding of gastric submucosal hematoma and improve the level of clinical treatment. Methods A total of 610 cases of gastroduodenal ulcer caused by upper gastrointestinal bleeding gastroscope archives re-read the picture to find 18 cases of gastric submucosal hematoma and 2 cases of confirmed gastric submucosal hematoma and clinical data, the relevant etiology Retrospective analysis. Results Gastric submucosal hematoma often accompanied by stressful life events, and stomach, duodenal ulcer and other primary disease. Therefore, submucosal hematoma may be stomach, duodenal ulcer complications, and more small hematoma (surface diameter less than 1cm) appear. Discussion Gastric submucosal hematoma is the stomach, duodenal ulcer under stress one of the complications. Due to small hematoma, the clinical atypical, difficult to identify or omission of observation. Stressful life events are one of the most direct causes of stomach, duodenal ulcer with hemorrhage and submucosal hematoma.
