头颅部闭合性创伤,尤其是额部或面部受碰击者,可以伤及视神经或视交叉。严重病例常致身亡。幸存者在视野检查中,可以出现双颞侧缺损,甚至双颞例偏盲,可助定位诊断。 本文报告3例头颅部闭合性创伤后查到齐正中线的颞侧视野缺损。这类外伤在临床工作中并非少见,而有关视野检查未得到应有的重视而漏诊。本文复习了有关文献,并对头颅闭合性创伤导致视交叉损伤的机理进行了讨论。
Closed head trauma, especially in frontal or facial bruises, can damage the optic nerve or optic chiasm. Serious cases often cause death. Survivors in the visual field examination, there may be bilateral temporal defect, or even bimodal hemianopsia, diagnosis can help position. This paper reports 3 cases of craniocerebral trauma after the detection of the median frontal temporal visual field defect. Such trauma in clinical work is not uncommon, and the relevant field of vision has not received due attention and missed. This article reviewed the literature and discussed the mechanism of optic chiasm injury caused by closed craniocerebral trauma.
