目的:探讨胃癌及癌前疾病患者血清细胞纤维连接蛋白(cFN)含量在不同胃疾病中变化情况及临床意义,及与幽门螺杆菌(HP)感染的相关性。方法:利用酶联免疫吸附试验检测1177例正常人、癌前疾病及胃癌患者血清cFN含量、胃疾病患者血清HP抗体含量。结果:血清cFN含量胃癌组明显高于正常人及胃良性疾病组(P<0.05),分化不良组显著高于分化良好组(P<0.01),淋巴结转移阴、阳性组比较差异无显著性(P>0.05)。胃疾病患者血清HP抗体检测,血清cFN含量,HP阴、阳性组比较差异无显著性,但在胃良性病变组中HP抗体阳性组较阴性组高,而胃癌组HP抗体阳性组较阴性组低。结论:血清cFN含量测定可用于胃良恶性疾病的鉴别,对胃癌诊断具有潜在的临床应用价值;胃癌患者血清cFN含量与胃癌分化程度密切相关,与胃淋巴结转移无关。血清cFN含量HP抗体阴性、阳性组比较差异无显著性,但胃良性疾病组,HP抗体阳性组较HP阴性组略高,胃癌组,HP抗体阳性组较HP阴性组略低,提示HP感染在胃良性疾病阶段和胃癌阶段,对血清cFN含量可能具有不同影响。由于血清HP抗体与其HP感染不一定平行,HP感染对血清cFN含量的影响有待于进一步深入探讨。
Objective: To investigate the changes and clinical significance of serum fibronectin (cFN) levels in patients with gastric cancer and precancerous lesions and their relationship with Helicobacter pylori (HP) infection. Methods: Serum cFN levels in 1177 normal subjects, precancerous lesions and gastric cancer patients and serum HP antibody levels in patients with gastric diseases were detected by enzyme-linked immunosorbent assay (ELISA). Results: The serum level of cFN in gastric cancer group was significantly higher than that in normal people and gastric benign disease group (P <0.05), poorly differentiated group was significantly higher than that in well differentiated group (P <0.01), lymph node metastasis was negative and positive group was no significant difference P> 0.05). There was no significant difference in serum HP antibody, serum cFN level, HP negative and positive group in patients with gastric diseases, but higher in HP positive group than in negative group in gastric benign lesion group, but lower in negative group than in HP positive group . Conclusions: Serum cFN levels can be used to identify benign and malignant gastric diseases and have potential clinical value in the diagnosis of gastric cancer. Serum cFN levels in gastric cancer patients are closely related to the degree of differentiation of gastric cancer, and have no relation with gastric lymph node metastasis. Serum cFN levels HP antibody negative, positive group was no significant difference, but gastric benign disease group, HP antibody positive group slightly higher than the HP negative group, gastric cancer group, HP antibody positive group slightly lower than the HP negative group, suggesting that HP infection in the Gastric benign disease stage and gastric cancer stage, the serum cFN content may have different effects. As the serum HP antibody and its HP infection are not necessarily parallel, the impact of HP infection on serum cFN content needs further study.
