目的观察阿托伐他汀钙对急性脑梗死(Acute cerebra infarction,ACI)后血白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平及预后的影响。方法分析我院自2010年1月至2012年1月收治的急性脑梗死患者74例,随机分为对照组35例给予常规治疗方法,观察组39例在常规治疗的基础上加用阿托他汀钙(20mg/次/d)。比较两组治疗前、治疗后3d,7dIL-6和TNF-α水平变化,以及两组治疗前后神经缺损功能(ESS)及日常生活能力Barthel指数(BI)的变化。结果①对照组与观察组治疗后3d,7dIL-6和TNF-α水平与治疗前比较,差异有统计学意义(P<0.05)。对照组与观察组治疗后3d,7dIL-6和TNF-α水平比较,差异有统计学意义(P<0.05)。②对照组与观察组治疗后3d,7dESS、BI水平与治疗前比较,差异有统计学意义(P<0.05)。对照组与观察组治疗后3d,7dESS、BI水平比较,差异有统计学意义(P<0.05)。结论阿托伐他汀钙可降低急性脑梗死患者血浆中IL-6与TNF-α水平,同时显著改善ESS、BI水平,表明阿托伐他汀钙用于急性脑梗死,具有减轻炎症反应促进预后的效果,值得临床推广。
Objective To observe the effect of atorvastatin calcium on the levels of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and prognosis in patients with acute cerebral infarction (ACI). Methods 74 patients with acute cerebral infarction who were admitted to our hospital from January 2010 to January 2012 were randomly divided into control group (n = 35) and conventional treatment group (n = 39). The observation group was treated with atorvastatin Calcium (20 mg / time / d). The levels of IL-6 and TNF-α in the two groups were compared before and after treatment, and the changes of neurological deficit function (ESS) and daily living ability Barthel index (BI) before and after treatment were compared between the two groups. Results ① The levels of IL-6 and TNF-α in the control group and the observation group after 3 and 7 days of treatment were significantly different from those before treatment (P <0.05). The levels of IL-6 and TNF-α in the control group and the observation group at 3d, 7d after treatment were significantly different (P <0.05). ② The levels of 3d, 7dESS and BI in the control group and the observation group after treatment were significantly different from those before treatment (P <0.05). There were significant differences in 3d, 7dESS and BI levels between the control group and the observation group after treatment (P <0.05). Conclusions Atorvastatin calcium can reduce the plasma levels of IL-6 and TNF-α in patients with acute cerebral infarction and at the same time significantly improve the levels of ESS and BI, indicating that atorvastatin calcium is effective in reducing acute inflammation and promoting prognosis Effect, it is worth clinical promotion.
