目的:音乐治疗联合舒芬太尼术后静脉镇痛用于剖宫产术后患者的焦虑心理,镇痛和血流动力学及住院天数的影响。方法:择期剖宫产手术患者200例,ASAⅠ—Ⅱ级,采用数字表法随机分为二组(n=100):音乐治疗组(M组)和对照组(C组)。M组患者术前接收音乐放松训练30m i n ,术后5,10,15,20,25,30h分别进行30m i n音乐干预。C组患者在同一时间段不采用任何音乐治疗。术后M组和C组患者均接受舒芬太尼静脉术后自控镇痛(postoperative paitent-controlled anaigesia,PCA).记录患者术后7,12,17,22,27,32h患者的焦虑自评量表(self rating anxiety scale,ASA)分值,视觉模拟评分(visual analogue scale,VAS),收缩压(systolic blood pressure,SBP),舒张压(diastolic blood pressure,DBP)和心率(heart rate,HR)变化。记录术后舒芬太尼用量,PCA按压次数以及不良反应;记录患者术后至出院的天数。结果:与C组相比,M组患者术后各时点的SAS评分,VAS评分,SBP,DBP,和HR均显著降低(P<0.01),M组的舒芬太尼用量和PCA按压次数明显减少(P<0.01),并且不良反应与住院天数均显著降低(P<0.01).结论:音乐联合舒芬太尼术后静脉镇痛用于剖宫产术后的患者能够达到更好的镇痛效果,且能够缓解患者的焦虑情绪,有效的稳定患者血流动力学的稳定并减少舒芬太尼的用量;明显的缩短住院天数。“,”Objective To evaluate the effects of musical therapy combined with sufentanil for postoperative analgesia after cesarean section in patients with anxiety, pain and hemodynamic impact and the number of days of hospitalization.Methods200 cases of elective cesarean surgery( ASAⅠ-Ⅱ grade)using a random number table, divided into two groups (n = 100): Musical therapy group (M group) and control group (group C). The patients receiving preoperative music relaxation training 30min, 30min after 5,10,15,20,25,30h music intervention were carried out. Group C patients do not use any music therapy at the same time. M and C group after patients received intravenous sufentanil postoperative analgesia (postoperative paitent-controlled anaigesia, PCA). Record after sufentanil dosage, PCA press times and adverse reactions; several days to discharge patients records.ResultsCompared with group C, group M patients at each time point of SAS score, VAS score, SBP, DBP, and HR were significantly lower (P <0.01), M group sufentanil dosage and PCA press times significantly reduced (P <0.01), and the adverse reactions and the number of days of hospitalization were significantly lower (P <0.01)ConclusionMusical therapy combined with sufentanil for postoperative analgesia after cesarean section patients can achieve better analgesic effect, and can alleviate patient anxiety, hemodynamic stability and reduce the amount of sufentanil effective in patients with stable; obviously shorten hospital stay.
