目的:总结 endobutton钢板重建喙锁韧带治疗肩锁关节脱位的临床疗效。方法Rockwood Ⅲ型以上肩锁关节脱位患者28例,其中16例采用endobutton钢板治疗(A组),12例采用锁骨钩钢板治疗(B组)。比较两组手术时间和术中出血量;术后采用 Karlsson标准评定肩关节功能。结果手术均获成功。A 组手术时间和术中出血量均少于B组[(56.9±17.0) min vs .(72.3±15.5) min和(66.3±27.5) ml vs .(90.0±24.9) ml](P<0.05)。术后3个月的Karlsson肩关节功能评定显示,A 组肩关节功能优良率高于 B 组(100% v s .75.0%)( P<0.05)。结论Endobutton钢板重建喙锁韧带技术具有植入物小、生物相容性好和固定可靠的优点;重建的生理性韧带不干扰肩峰下间隙和肩关节活动,有助于患者早期肩关节功能锻炼和恢复。“,”Objective To summarize clinical efficacy of endobutton plate in treating acromioclavicular joint dislocation .Methods Twenty‐eight patients with acromioclavicular joint dislocation of type Rockwood Ⅲ or more were treated with endobutton plate(group A ,16 cases) and clavicular hook plate(group B ,12 cases) .The operation time and blood loss were compared .Shoulder function in three months after surgery was evaluated by Karlsson standards .Results The operation was successful in all patients .The operation time and blood loss were less in group A than those in group B[(56.9 ± 17.0) min vs .(72.3 ± 15.5) min and (66.3 ± 27.5) ml vs .(90.0 ± 24.9) ml] (P<0 .05) .The excellent and good rate in the 3rd month after operation was higher in group A than that in group B(100% vs .75.0% )(P<0 .05) .Conclusion Endobutton coracoclavicular ligament reconstruction technique has the advantages of small objects implanted ,good biocompatibility and reliable fixation .The physiological coracoclavicular ligament reconstructed with endobutton plate does not interfere the subacromial space and activity of shoulder joint ,which benefits early shoulder exercise and rehabilitation of the acromioclavicular joint .
