目的：介绍低体重小儿先天性心脏病的外科治疗体会。方法：自１９９０年６月～１９９７年６月，进行低体重小儿先天性心脏病Ⅰ期根治手术１０３例，体重８－１５ｋｇ，年龄１．４－８岁，其中合并中度以上肺动脉高压５４倒，复杂先天性心脏病２５例。经右腋下小切口在常温体外循环心脏跳动下手术９例，经胸部正中切口在中度低温体外循环下手术９１例，深低温低流量体外循环下手术３例。体外循环时间（５６．４±２０．６）ｍｉｎ，主动脉阻断时间（２７．５±１１．７）ｍｉｎ。结果：住院死亡６例，存活９７例，随访２月～３年，无并发症和死亡，心功能Ⅰ级。结论：低体重小儿常见先天性心脏病行Ⅰ期根治手术是可行的，效果良好；经右腋下小切口进行选择性心内直视手术简便安全、美学效果好。
Objective: To introduce the surgical treatment of congenital heart disease in children with low birth weight. Methods: From June 1990 to June 1997, 103 cases of low-weight pediatric congenital heart disease undergoing radical surgery were performed in 103 cases with a body weight of 8-15kg and a mean age of 1.4-8 years. 54 cases of pulmonary hypertension , Complicated congenital heart disease in 25 cases. 9 cases underwent cardiac arrhythmia with right axillary incision at room temperature and 91 cases underwent moderate hypothermic cardiopulmonary bypass through the mid-thoracic incision, and 3 cases under deep hypothermia and low-flow cardiopulmonary bypass. The time of cardiopulmonary bypass (56.4 ± 20.6) min and the time of aortic block (27.5 ± 11.7) min. Results: There were 6 hospitalized deaths and 97 survived. The patients were followed up for 2 months to 3 years without complications and death, and their cardiac function was grade Ⅰ. Conclusion: The first-stage radical operation of common congenital heart disease in children with low birth weight is feasible and effective. The selective small open right armpit for selective open heart surgery is simple and safe, and has a good aesthetic effect.
