目的采用脑循环动力学检测手段对脑动脉硬化症的诊断价值进行探讨。方法选择脑动脉硬化症患者28例(病例组)。其中男性16例,女性12例。平均年龄48.6岁。对病例组患者进行脑循环动力学检测。所得指标:(1)CVDIm运动学参数;平均血流量(Qmean)、最大血流量(Qmax)、最小血流量(Qmin)、平均血流速度(Vmean)、最大血流速度(Vmax)、最小血流速度(Vmin);(2)CVDI动力学参数:外周阻力(Rv)、特性阻抗(Zc)、动态阻力(DR)、脉搏波动(Wv)、动脉弹性模量(E)、血管顺应性(C)和临界压力(CP)。并将所得CVDI指标与性别、年龄等相应的正常值(对照值)进行比较。然后,对两组数据进行统计学处理、结果两组间CVDI运动学指标无显著差异(P>0. 05);反映脑血管弹性的动力学指标(包括Wv、Zc、C),两组间差异非常显著(P<0. 001)、表明;病例组患者虽然在脑血流量、血流速度方面大体上与对照组持平,但因血管弹性较差,可能直接影响其自身的脑血管代偿机能。这有利于解释脑动脉硬化症患者多有持久而固定的高级神经活动功能下降,且随脑力或体力活动增加而出现间歇性临床症状加重等特点、结论CV?
Objective To investigate the diagnostic value of cerebral arteriosclerosis by means of cerebral circulation kinetics. Methods Select 28 patients with cerebral arteriosclerosis (case group). There were 16 males and 12 females. The average age of 48.6 years old. Patients in the case group were subjected to cerebral circulation kinetic test. The obtained index: (1) CVDIm kinetic parameters; Qmean, Qmax, Qmin, Vmean, Vmax, (Vmin); (2) CVDI kinetic parameters: Rv, Zc, DR, Wv, E, Vascular compliance C) and critical pressure (CP). The CVDI index was compared with the normal (control) value corresponding to sex, age, and the like. Then, the data of two groups were statistically analyzed. There was no significant difference in the kinematic indexes of CVDI between the two groups (P> 0.05). The kinetic indexes (including Wv, Zc and C) The difference was very significant (P <0.001), indicating that although the patients in the group of cerebral blood flow velocity and blood flow were generally flat with the control group, but due to poor blood vessel elasticity may have a direct impact on their own cerebral vascular compensation function. This is conducive to explain the characteristics of patients with cerebral arteriosclerosis more persistent and fixed advanced neurological decline, and with the increase of mental or physical activity and the emergence of intermittent clinical symptoms and other characteristics of the conclusion CV?
