目的 应用流式细胞仪(FCM)测定食管癌患者单克隆抗体(McAb)标记的淋巴细胞亚群比例,利用~3H—胸腺嘧啶核苷(~3H—TdR)和~(14)C—尿嘧啶核苷(~(14)C—UR)作掺入试验测定T、B细胞的转化功能。方法 采用双夹心ELISA法测定血清可溶性白细胞介素—2受体(sIL—2R)的血清值。结果 食管癌患者淋巴细胞亚群比例失调。CD_4~+显著降低(P<0.01),CD_8~+,CD_(19)~+(B细胞)显著上升(P<0.05),CD_4~+/CD_8~+下降明显(P<0.01),放疗更加剧了这种紊乱(P<0.01),CD_4~+/CD_8~+倒置。T细胞DNA和RNA合成能力显著下降(P<0.01)而B细胞显著增强(P<0.01),放疗进一步增加了这种差别(P<0.01)。血清sIL—2R水平显著高于正常(P<0.01),而放疗结束,SIL—2R水平接近正常(P<0.01)。结论 食管癌患者免疫紊乱,放疗更进一步使细胞免疫降低,体液免疫增强。可能是食管癌局部难以控制的机制之一。
Objective To use flow cytometry (FCM) to determine the proportion of monoclonal antibody (McAb)-labeled lymphocyte subsets in patients with esophageal cancer, using ~3H-thymidine (~3H-TdR) and ~(14)C-uracil Nucleosides (~(14)C-UR) were used for the incorporation test to determine the conversion function of T and B cells. Methods The serum levels of serum soluble interleukin-2 receptor (sIL-2R) were determined by double sandwich ELISA. Results The proportion of lymphocyte subsets in patients with esophageal cancer was imbalanced. CD_4~+ decreased significantly (P<0.01), CD_8~+, CD_(19)~+ (B-cells) increased significantly (P<0.05), and CD_4~+/CD_8~+ decreased significantly (P<0.01). This disorder was exacerbated (P<0.01), and CD4+/CD8+ was inverted. The ability of T cells to synthesize DNA and RNA was significantly decreased (P<0.01) and B cells were significantly increased (P<0.01), and radiotherapy further increased this difference (P<0.01). The level of serum sIL-2R was significantly higher than normal (P<0.01), and the end of radiotherapy, SIL-2R levels were close to normal (P<0.01). Conclusion Immune disorders in patients with esophageal cancer, radiotherapy further reduce the cellular immunity, humoral immunity. It may be one of the mechanisms that are difficult to control locally in esophageal cancer.
