目的分析我院静脉药物配置过程中常见不合理用药情况,并提出不合理用药的防范措施。方法对我院2010年5月至2011年5月静脉药物配置过程中药师遇到的不合理用药医嘱进行统计分析。结果共审查出不合理用药医嘱213例(占0.36%),主要包括给药方法、配伍禁忌、给药剂量、给药浓度、溶媒选择以及电脑录入等方面的问题。结论药师通过静脉药物配置及时发现并纠正临床用药不合理现象,有效减少了临床因用药不合理引起的不良事件,充分发挥了药师的价值,促进患者合理用药。
Objective To analyze the common irrational use of drugs in the process of intravenous drug allocation in our hospital and to put forward the countermeasures of irrational drug use. Methods The statistical analysis of irrational medical doctor’s orders encountered by pharmacists in the process of intravenous drug allocation in our hospital from May 2010 to May 2011 was statistically analyzed. Results A total of 213 cases (0.36%) of unreasonable medical orders were examined, including the administration methods, compatibility taboo, dosage, drug concentration, solvent selection and computer input. Conclusions Pharmacists find out and correct the irrational clinical use of drugs through intravenous drug allocation in time, effectively reduce the adverse events caused by irrational use of drugs in clinical practice, give full play to the value of pharmacists and promote the rational use of drugs in patients.
