目的观察糖尿病(DM)肾动脉的血流动力学的改变,探讨彩色多普勒超声诊断糖尿病肾病(DN)的临床价值。方法应用彩色多普勒血流显像对45例2型糖尿病肾病(DN)患者的段动脉(SRA)、叶间动脉(IRA)、弓形动脉(ARA)进行检查,并对其血流的收缩期最大血流速度(Vsmax)、舒张期最低血流速度(Vdmin)和搏动指数(PI)、阻力指数(RI)进行对比分析。结果随着肾脏的损害,患者肾血流多普勒频谱特点是:SRA、IRA、ARA的Vsmax、Vdmin,PI、RI增高。尤其当RI大于0.7,肾脏将出现早期损害。结论彩色多普勒超声检测肾血流参数在预测糖尿病肾病肾脏损害方面有一定临床参考价值,为糖尿病肾病的诊断提供了一种简便的方法。
Objective To observe the hemodynamic changes of diabetic renal artery (DM) and to explore the clinical value of color Doppler ultrasonography in the diagnosis of diabetic nephropathy (DN). Methods Color Doppler flow imaging was used to examine the segmental arteries (SRA), the interlobar arteries (IRA) and the arcuate arteries (ARA) in 45 patients with type 2 diabetic nephropathy (DN) Vsmax, Vdmin, PI and RI were compared between the two groups. Results With the damage of the kidneys, the Doppler spectral characteristics of renal blood flow were: Vsmax, Vdmin, PI, RI of SRA, IRA and ARA increased. Especially when RI is greater than 0.7, the kidneys will show early damage. Conclusion The color Doppler ultrasound detection of renal blood flow parameters in the prediction of diabetic nephropathy kidney damage has some clinical value for the diagnosis of diabetic nephropathy provides a simple method.
