目的探讨B型利钠多肽(BNP)与影响充血性心力衰竭(CHF)严重程度及预后各因素间的相关性。方法对179例患者进行血浆N末端BNP原(NT-proBNP)及血压、血糖、血清肌酐、尿素氮、胸片的测定,并根据性别、年龄、左室射心血分数(EF)、血压水平分组。通过心脏彩色多普勒超声检查,测得79例心衰事件发生患者72小时内的EF。结果进行统计检验,以评价NT-proBNP与影响心功能衰竭的相关因素年龄、性别、EF、血压间的关系。结果CHF患者组的NT-proBNP浓度与年龄存在着正相关关系,很可能的舒张性心力衰竭患者(DHF)组NT-proBNP浓度与年龄间也存在着正相关关系。EF与NT-proBNP浓度间表现出负相关关系。在不同性别组间NT-proBNP浓度存在着显著性差异,但相同的现象在CHF组和很可能的DHF组患者中未被发现。血压水平与NT-proBNP浓度间未发现存在相关关系。结论NT-proBNP浓度检测与年龄和EF关系明确。在CHF患者中,NT-proBNP浓度与性别、血压水平、心房纤颤及其它影响因素间的关系还应当进行更深入,样本量更大的研究。
Objective To investigate the relationship between the B-type natriuretic peptide (BNP) and the severity and prognosis of congestive heart failure (CHF). Methods The plasma levels of N-terminal proBNP and blood pressure, blood glucose, serum creatinine, blood urea nitrogen, and chest radiographs were measured in 179 patients. According to gender, age, left ventricular ejection fraction (EF) and blood pressure . Through the heart color Doppler ultrasound examination, 79 cases of heart failure occurred in patients within 72 hours of EF. The results of statistical tests to assess the impact of NT-proBNP and factors related to heart failure age, gender, EF, blood pressure. Results There was a positive correlation between NT-proBNP concentration and age in CHF patients. There was also a positive correlation between NT-proBNP concentration and age in probable patients with diastolic heart failure (DHF). EF and NT-proBNP concentration showed a negative correlation between. There was a significant difference in NT-proBNP concentrations between different sex groups, but the same phenomenon was not found in the CHF group and in the probable DHF group. No correlation was found between blood pressure levels and NT-proBNP concentrations. Conclusion The detection of NT-proBNP concentration is clearly related to age and EF. The relationship between NT-proBNP concentration and gender, blood pressure levels, atrial fibrillation and other influencing factors in CHF patients should also be investigated in greater depth and with larger sample sizes.
