对81例胸腔积液患者测定了胸水腺苷脱氨酶(ADA)活性,其中结核性胸水55例、恶性胸水22例和漏出性胸水4例。各组ADA活性分别为188.3±58.2u、52.2±15.4u和15.0±10.5u。结核组胸水ADA活性明显高于恶性组(P<0.001),两组病例间很少重叠。在结核组中ADA>110u者53例(96.3%),>120u者52例(94.5%);恶性组无一例>90u,其中<70u者20例(90.9%)。漏出液组胸水.ADA值明显低于结核组和恶性组,差异非常显著(P<0.001)。结果表明:胸水ADA活性可作为鉴别结核性与恶性胸水的有用指标。
The activity of adenosine deaminase (ADA) in pleural effusion was measured in 81 patients with pleural effusion, including 55 cases of tuberculous pleural effusion, 22 cases of malignant pleural effusion and 4 cases of leakage pleural effusion. The ADA activity in each group was 188.3±58.2u, 52.2±15.4u and 15.0±10.5u, respectively. The activity of ADA in pleural effusion in the tuberculosis group was significantly higher than that in the malignant group (P<0.001). There was little overlap between the two groups of cases. In the tuberculosis group, there were 53 cases (96.3%) with ADA>110u, and 52 cases (94.5%) with>120u; none of the malignant group>90u, and 20 cases (90.9%) with <70u. The pleural effusion and ADA values in the leakage fluid group were significantly lower than those in the tuberculosis and malignant groups (P<0.001). The results showed that the ADA activity of pleural effusion can be used as a useful indicator to identify tuberculous and malignant hydrothorax.
