目的评价局部放疗配合放射性核素89Sr治疗骨转移癌的疗放,分析单纯放疗单纯核素89Sr治疗及联合治疗的副作用。方法观察60例确诊为骨转移癌的患者,分为3组,每组20例。局部放疗(A组),采用6MV直线加速器外照射给予吸收剂量30~60Gy/2~4周,局部放疗+89Sr治疗(B组),单纯89Sr治疗(C组)放射性核素89Sr静脉内注射3~4mCi。结果治疗后B组骨痛缓解,原发灶改善明显好于A、C组;新增疼痛部位及转移灶均少于A、C组(P<0·05)。治疗后血液学的毒性3组差异统计学意义(P>0·05)。结论局部放疗配合89Sr治疗骨转移癌有较好的疗效,提示对单发性骨转移患者的压痛明显部位给予局部外照射止痛效果明显,对多发骨转移且病灶相距较近的给予放疗联合89Sr治疗效果安全可靠,对全身性多发性骨转移者采用单纯放射性核素89Sr治疗对于止痛和控制骨转移有一定疗效。
Objective To evaluate the efficacy of local radiotherapy combined with radionuclide 89Sr in the treatment of bone metastases and to analyze the side effects of 89Sr radiotherapy alone combined with radiotherapy. Methods Sixty patients with bone metastases were observed and divided into three groups of 20 patients. Local radiotherapy (group A) was administered by external beam irradiation with a 6MV linear accelerator for 30-60 Gy / 2-4 weeks, radiotherapy + 89Sr (group B), and 89Sr treatment alone (group C). Radionuclide 89Sr was injected intravenously 3 ~ 4mCi. Results After treatment, the pain in group B was relieved, and the improvement in primary lesions was significantly better than that in groups A and C. The new pain sites and metastases were less than those in groups A and C (P <0.05). The hematological toxicities after treatment were statistically different among the three groups (P> 0.05). Conclusion The combination of local radiotherapy and 89Sr for the treatment of metastatic bone cancer has a good curative effect. It is indicated that the external pain reliever is obviously given to the site of tender tenderness in patients with solitary bone metastasis, and radiotherapy combined with 89Sr The effect is safe and reliable, the use of pure radionuclide 89Sr treatment for patients with systemic multiple bone metastasis for pain relief and bone metastasis control have a certain effect.
