目的:研究鸦胆子油乳注射液对大肠癌患者的临床疗效及可能机制。方法:选取南京医科大学附属江苏省肿瘤医院2014年7月~2016年4月收治的结肠癌及直肠癌患者100例,采用随机、对照、单盲研究方法,随机分为观察组和对照组,对照组患者采用化疗方案:第1天静脉滴注奥沙利铂120 mg/m~2,第1~5天静脉滴注亚叶酸钙200 mg/m~2和5-氟尿嘧啶300 mg/m~2,每隔3周重复上述疗程。观察组在化疗方案基础上加用鸦胆子油乳注射液30 mL加入生理盐水250 mL中静脉滴注,1次/天,两组均治疗3个月,比较两组治疗后临床疗效及不良反应发生情况。结果:治疗后观察组血清IgG、IgA、IgM均显著高于对照组(P<0.05);外周血CD3、CD4~+、CD8~+、CD4~+/CD8~+显著高于对照组(P<0.05);总有效率及生活质量评分显著高于对照组(P<0.05);恶心、呕吐、食欲减退等不良反应程度显著低于对照组(P<0.05)。结论:鸦胆子油注射液能通过增强大肠癌患者免疫系统改善大肠癌患者病情并减轻化疗药物不良反应,提高患者的生活质量。
Objective: To study the clinical effect and possible mechanism of Brucea javanica oil emulsion in patients with colorectal cancer. Methods: One hundred patients with colon cancer and rectal cancer admitted to Jiangsu Tumor Hospital Affiliated to Nanjing Medical University from July 2014 to April 2016 were randomly divided into observation group and control group with randomized, controlled and single-blind study. Patients in the control group received chemotherapy with oxaliplatin 120 mg / m 2 intravenously on day 1, leucovorin 200 mg / m 2 and 5- fluorouracil 300 mg / m 2 on day 1 to day 5, 2, repeat the course of treatment every 3 weeks. Observation group based on chemotherapy plus javanica oil emulsion injection 30 mL 250 mL normal saline intravenously, once daily, both groups were treated for 3 months, the clinical efficacy and adverse reactions were compared between the two groups after treatment What happened? Results: The levels of IgG, IgA and IgM in the observation group were significantly higher than those in the control group (P <0.05). The levels of CD3, CD4 +, CD8 +, CD4 + / CD8 + in peripheral blood in the observation group were significantly higher than those in the control group <0.05). The total effective rate and quality of life scores were significantly higher than those in the control group (P <0.05). The adverse reactions such as nausea, vomiting and loss of appetite were significantly lower than those in the control group (P <0.05). Conclusion: Brucea javanica oil injection can improve the condition of patients with colorectal cancer by decreasing the immune system of patients with colorectal cancer and relieve the side effects of chemotherapy drugs and improve the quality of life of patients.
