目的:了解单一高危型人乳头瘤病毒(HR-HPV)感染而无宫颈细胞病变妇女的自然转归,探讨宿主自然清除HPV病毒感染的相关因素。方法:选取单一HR-HPV感染、无宫颈细胞病变且随访资料完整的妇女149例,知情同意后,每3月随访1次,连续随访2年,统计分析HPV自然清除的时间和相关因素。结果:单一HR-HPV感染者149例,自然清除时间为7.7±5.3月,其中1年内自然清除率占79.2%(118/149),2年内自然清除率占88.6%(132/149)。不同年龄组(20~29岁、30~39岁、40~49岁、≥50岁)的妇女单一HR-HPV感染清除平均时间分别为7.12±4.23月、7.44±5.86月、8.32±5.09月和11.50±8.89月,4组比较,差异有统计学意义(P<0.05),感染者年龄与自然清除HPV病毒时间呈正相关关系(r=0.871,P=0.023),而初诊HPV检测值与HPV病毒清除时间无相关性(r=0.106,P=0.192)。结论:单一HR-HPV感染而无宫颈细胞病变的妇女,大部分2年内能够自然清除HPV感染;年龄与自然清除HPV病毒时间相关。姑息或期待处置,可减少药物或物理治疗的使用。
Objective: To understand the natural outcome of women with single high-risk human papillomavirus (HR-HPV) infection but without cervical cell disease, and to explore the host-related factors that naturally eradicate HPV infection. Methods: One hundred and ninety-nine women with complete HR-HPV infection without cervical lesions and complete follow-up were enrolled. After informed consent, the patients were followed up every three months and followed up for 2 years. The time and related factors of natural HPV clearance were statistically analyzed. Results: 149 patients with single HR-HPV infection had a spontaneous clearance time of 7.7 ± 5.3 months, of which 79.2% (118/149) were natural clearance in one year and 88.6% (132/149) in 2 years. The average time of single HR-HPV infection in women of different age groups (20-29, 30-39, 40-49, ≥50) were 7.12 ± 4.23 months (P <0.05). There was a positive correlation between age of infected persons and time of spontaneous clearance of HPV (r = 0.871, P = 0.023) There was no correlation between clearance time (r = 0.106, P = 0.192). CONCLUSIONS: Women with a single HR-HPV infection but without cervical lesions are naturally cleared of HPV infection within 2 years; age is naturally associated with HPV clearance time. Palliative or expectant treatment can reduce the use of medications or physical therapy.
