目的分析福建省2004—2009年甲型病毒性肝炎(甲肝)的流行病学特征,为制定防控策略提供依据。方法利用《疾病监测信息报告管理系统》监测报告卡信息,用SAS 9.0软件对数据进行统计分析。结果 2004—2009年全省共报告甲肝8 778例,死亡2例,年均发病率4.10/10万,总体呈逐年下降趋势;实验室年均检出率53.7%,呈逐年上升趋势。各地市均有疫情,以宁德最高,厦门最低;2009年发病率莆田最高;疫情均散发。各年度疫情总体以3月为峰值,2～5月为发病高峰期,呈单峰分布,8月出现小反弹。发病率男女之比2.34∶1,发病年龄为出生18 d至98岁,各年龄组均有病例,高发人群为25～59岁组(合计59.0%),70～74岁组发病率最高;人群以农民为主(44.4%)。结论福建省甲肝发病率逐年下降,防控工作卓有成效,但疫情高发县区发生转移,提示应结合疫情特征的变化来加强防控力度。 Objective To analyze the epidemiological characteristics of hepatitis A virus in Fujian Province from 2004 to 2009 and provide the basis for the development of prevention and control strategies. Methods The disease monitoring information report management system was used to monitor the report card information and SAS 9.0 software was used to analyze the data. Results A total of 8 778 cases of hepatitis A were reported in the province between 2004 and 2009, with 2 deaths, the annual average incidence was 4.10 / 100 000, showing an overall decreasing trend year by year. The average annual detection rate in the laboratory was 53.7%, showing an upward trend year by year. All cities have epidemics, with the highest Ningde, Xiamen lowest in 2009, the highest incidence of Putian; outbreaks are distributed. The epidemic overall in each year in March as the peak, 2 to May peak incidence, showing a single peak distribution, a small rebound in August. The incidence of male to female ratio was 2.34: 1, and the age of onset was from 18 to 98 years of age, with all age groups. The highest incidence was 25-59 years old (59.0% of total), while the highest incidence was found in 70-74 years old. Mainly farmers (44.4%). Conclusions The incidence of hepatitis A in Fujian Province declines year by year, and the prevention and control work is very effective. However, the metastasis occurs in the high incidence counties, suggesting that the intensity of prevention and control should be strengthened in light of the change of epidemic characteristics.
