目的:通过对吸烟相关喉鳞状细胞癌患者乙酰肝素酶(HPA)及NF-E2相关因子(NRF2)的表达情况的分析,进一步探讨二者对吸烟相关喉鳞状细胞癌的预后影响。方法:连续性收集2014-2015年84例喉鳞状细胞癌患者的临床资料,根据吸烟指数分为3组:重度吸烟组30例、轻中度吸烟组20例及无吸烟组34例。分别检测3组患者血清抗氧化指标及喉癌组织HPA和NRF2的表达水平,结合患者临床资料探讨其与患者预后及临床分期之间的关系。结果:1Ⅲ~Ⅳ期喉鳞状细胞癌患者癌组织中HPA及NRF2均呈高表达;Ⅰ~Ⅱ期喉鳞状细胞癌患者癌组织中HPA及NRF2均呈低表达。2重度吸烟组NRF2表达水平显著高于轻中度吸烟组和无吸烟组(均P<0.05),且在分期较高的喉鳞状细胞癌患者中其NRF2表达水平均高于分期较低者;3HPA及NRF2的表达与喉鳞状细胞癌患者的临床分期有关(P<0.05)。结论:与非吸烟患者比较,重度吸烟喉鳞状细胞癌患者面临更加严重的氧化应激压力,喉鳞状细胞癌患者HPA及NRF2的表达水平与其临床病理分期有关,HPA及NRF2高表达可能促使喉鳞状细胞癌患者临床分期升级。
Objective: To further investigate the prognosis of smoking-related laryngeal squamous cell carcinoma (LSCC) by analyzing the expression of heparanase (HPA) and NF-E2 related factors (NRF2) in smoking-related laryngeal squamous cell carcinoma. Methods: The clinical data of 84 patients with laryngeal squamous cell carcinoma from 2014 to 2015 were collected. According to the smoking index, they were divided into 3 groups: 30 in severe smoking group, 20 in mild and moderate smoking group and 34 in non-smoking group. The serum anti-oxidant index and the expression of HPA and NRF2 in laryngeal carcinoma were detected in three groups of patients, and the relationship between prognosis and clinical stage were analyzed according to the clinical data of the patients. Results: HPA and NRF2 were highly expressed in the cancer tissues of stage Ⅲ ~ Ⅳ laryngeal squamous cell carcinoma, while the expression of HPA and NRF2 in stage Ⅰ ~ Ⅱ laryngeal squamous cell carcinoma was low. The expression of NRF2 in severe smoking group was significantly higher than that in mild to moderate smoking group and no smoking group (all P <0.05), and the expression level of NRF2 in laryngeal squamous cell carcinoma with higher stage was higher than that in lower stage The expression of 3HPA and NRF2 was related to the clinical stage of laryngeal squamous cell carcinoma (P <0.05). Conclusion: Compared with nonsmokers, severe smoking laryngeal squamous cell carcinoma patients face more severe oxidative stress. The expression of HPA and NRF2 in laryngeal squamous cell carcinoma is related to the clinicopathological stage. High expression of HPA and NRF2 may promote Laryngeal squamous cell carcinoma clinical staging upgrade.
